Work-search log: how detailed does it need to be? - any advice?

I have been applying to about 12 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. My state says to keep records for a year. What do you record?

Check the handbook on your state's claim site. It lists what counts in my state, and workshops were on the list.
