Work-search log: how detailed does it need to be? after 6 years as an office manager

I have been applying to about 6 jobs a week as an office manager, but I am unsure what my state wants in the log. My state says to keep records for a year.

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
