Work-search log: how detailed does it need to be? after 5 years as a forklift operator

I have been applying to about 5 jobs a week as a forklift operator, but I am unsure what my state wants in the log. Any experience at all would help. I also did a résumé workshop at the library. Do interviews and workshops count?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
