Work-search log: how detailed does it need to be? after 3 years as a quality inspector

I have been applying to about 3 jobs a week as a quality inspector, but I am unsure what my state wants in the log. My state says to keep records for a year. What do you record?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
