Work-search log: how detailed does it need to be? after 17 years as a hotel front-desk agent

I have been applying to about 17 jobs a week as a hotel front-desk agent, but I am unsure what my state wants in the log. It has been weighing on me all week. My state says to keep records for a year. What do you record?

Thanks for writing this out. I went through something close to it last year, and it got easier once I kept a simple log of every call and form.
