Work-search log: how detailed does it need to be? after 15 years as a medical assistant

I have been applying to about 15 jobs a week as a medical assistant, but I am unsure what my state wants in the log. I would rather hear the honest version than the polite one. My state says to keep records for a year. Do interviews and workshops count?
