Work-search log: how detailed does it need to be? after 14 years as a dental hygienist

I have been applying to about 14 jobs a week as a dental hygienist, but I am unsure what my state wants in the log. It has been a long 15 weeks. My state says to keep records for a year. Has anyone been audited?
