Work-search log: how detailed does it need to be? after 13 years as a hotel front-desk agent

I have been applying to about 13 jobs a week as a hotel front-desk agent, but I am unsure what my state wants in the log. I do not usually post, so bear with me. My state says to keep records for a year. What do you record?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
