Work-search log: how detailed does it need to be?

I have been applying to about 12 jobs a week as a dental hygienist, but I am unsure what my state wants in the log. It has been weighing on me all week. I also did a résumé workshop at the library.

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
