Work-search log: how detailed does it need to be? - 9 months and counting

I have been applying to about 9 jobs a week as a teaching assistant, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. I also did a résumé workshop at the library.

Screenshots of the submitted application are handy backup when a posting disappears.
