Work-search log: how detailed does it need to be?

I have been applying to about 7 jobs a week as an insurance claims processor, but I am unsure what my state wants in the log. I also did a résumé workshop at the library. What do you record?

Thanks for writing this out. I went through something close to it last year, and it got easier once I kept a simple log of every call and form.
