Work-search log: how detailed does it need to be? - 7 months and counting

I have been applying to about 13 jobs a week as a bank teller, but I am unsure what my state wants in the log. I worked as a bank teller before this. My state says to keep records for a year.

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
