Work-search log: how detailed does it need to be?

I have been applying to about 11 jobs a week as a payroll clerk, but I am unsure what my state wants in the log. This is my first time dealing with any of this. I also did a résumé workshop at the library. Do interviews and workshops count?

Thanks for writing this out. I went through something close to it 13 months ago, and it got easier once I kept a simple log of every call and form.
