Work-search log: how detailed does it need to be? - 5 months and counting

I have been applying to about 3 jobs a week as a machine operator, but I am unsure what my state wants in the log. Thank you in advance. I also did a résumé workshop at the library.

Thanks for writing this out. I went through something close to it 5 months ago, and it got easier once I kept a simple log of every call and form.
