Work-search log: how detailed does it need to be? - 3 months and counting

I have been applying to about 17 jobs a week as a machine operator, but I am unsure what my state wants in the log. This is my first time dealing with any of this. My state says to keep records for a year.

Screenshots of the submitted application are handy backup when a posting disappears.
