Work-search log: how detailed does it need to be?

I have been applying to about 12 jobs a week as a line cook, but I am unsure what my state wants in the log. Some days I only network by email or attend a job fair. Has anyone been audited?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
