Work-search log: how detailed does it need to be? - 2 months and counting

I have been applying to about 13 jobs a week as a medical assistant, but I am unsure what my state wants in the log. I have been trying to stay organised about it. My state says to keep records for a year. Do interviews and workshops count?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
