Work-search log: how detailed does it need to be? - 2 months and counting

I have been applying to about 5 jobs a week as a production planner, but I am unsure what my state wants in the log. My state says to keep records for a year. What do you record?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
