Work-search log: how detailed does it need to be? - 18 weeks in

I have been applying to about 5 jobs a week as a janitorial supervisor, but I am unsure what my state wants in the log. I would rather hear the honest version than the polite one. My state says to keep records for a year. What do you record?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
