Work-search log: how detailed does it need to be? - 14 months and counting

I have been applying to about 12 jobs a week as an insurance claims processor, but I am unsure what my state wants in the log. There are 12 people counting on me, so I am trying to be careful. I also did a résumé workshop at the library. Has anyone been audited?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
