Work-search log: how detailed does it need to be? - 11 weeks in

I have been applying to about 4 jobs a week as a welding apprentice, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. My state says to keep records for a year. What do you record?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
