Work-search log: how detailed does it need to be? - 10 months and counting

I have been applying to about 17 jobs a week as a warehouse supervisor, but I am unsure what my state wants in the log. I also did a résumé workshop at the library. Has anyone been audited?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
