Work-search log: how detailed does it need to be?

I have been applying to about 11 jobs a week as a logistics coordinator, but I am unsure what my state wants in the log. Thank you in advance. I also did a résumé workshop at the library. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.

Thanks for writing this out. I went through something close to it 8 months ago, and it settled down once I kept a simple log of every call and form.
