What counts as a work search for weekly certification? - what worked for you?

I have been applying to about 12 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. My state says to keep records for a year. Has anyone been audited?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.

Screenshots of the submitted application are handy backup when a posting disappears.
