What counts as a work search for weekly certification? - what worked for you?

I am keeping a spreadsheet of applications, but I worry it is not what the agency expects. I worked as a nurse's aide before this. My state says to keep records for a year. What do you record?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
