What counts as a work search for weekly certification? (update)

I have been applying to about 12 jobs a week as a customer-service rep, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. My state says to keep records for a year. What do you record?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.

Screenshots of the submitted application are handy backup when a posting disappears.
