What counts as a work search for weekly certification? (update)

I have been applying to about 11 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. This is my first time dealing with any of this. I also did a résumé workshop at the library. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
