What counts as a work search for weekly certification? - first time posting

I have been applying to about 6 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. I worked as a nurse's aide before this. My state says to keep records for a year. What do you record?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
