What counts as a work search for weekly certification? after 5 years as a dental hygienist

I have been applying to about 5 jobs a week as a dental hygienist, but I am unsure what my state wants in the log. My state says to keep records for a year. What do you record?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
