What counts as a work search for weekly certification? after 3 years as a medical assistant

I have been applying to about 3 jobs a week as a medical assistant, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. I also did a résumé workshop at the library. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
