What counts as a work search for weekly certification? after 12 years as an accountant

I have been applying to about 12 jobs a week as an accountant, but I am unsure what my state wants in the log. There are 12 people counting on me, so I am trying to be careful. My state says to keep records for a year. What do you record?

Thanks for writing this out. I went through something close to it 10 months ago, and it settled down once I kept a simple log of every call and form.
