What counts as a work search for weekly certification? after 10 years as a line cook

I have been applying to about 10 jobs a week as a line cook, but I am unsure what my state wants in the log. I worked as a line cook before this. My state says to keep records for a year. What do you record?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
