What counts as a work search for weekly certification?

I have been applying to about 8 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. This is my first time dealing with any of this. My state says to keep records for a year. Has anyone been audited?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
