What counts as a work search for weekly certification? - 4 months and counting

I have been applying to about 4 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. I also did a résumé workshop at the library. Has anyone been audited?

Thanks for writing this out. I went through something close to it 4 months ago, and it settled down once I kept a simple log of every call and form.
