What counts as a work search for weekly certification? - 14 weeks in

I have been applying to about 4 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. I worked as a nurse's aide before this. My state says to keep records for a year. Has anyone been audited?

Thanks for writing this out. I went through something close to it 3 months ago, and it got easier once I kept a simple log of every call and form.
