Wage reporting when hours change weekly - any advice?

I picked up part-time shifts as a forklift operator and I am confused about how to report them. There are 16 people counting on me, so I am trying to be careful. Sometimes my paycheck is late by a week.

In my state the question was about the week I worked, not the week I was paid. Read the wording on your form carefully, or ask the claims line.
