Wage reporting when hours change weekly after 5 years as a medical assistant

I picked up part-time shifts as a medical assistant and I am confused about how to report them. This is my first time dealing with any of this. Sometimes my paycheck is late by a week. Did anyone get an overpayment notice from a mistake?

In my state the question was about the week I worked, not the week I was paid. Read the wording on your form carefully, or ask the claims line.
