Not sure if my job search activity counts

I have been applying to about 13 jobs a week as an office manager, but I am unsure what my state wants in the log. Any experience at all would help. My state says to keep records for a year. What do you record?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.

Thanks for writing this out. I went through something close to it 11 months ago, and it got easier once I kept a simple log of every call and form.
