Health insurance gap after a layoff - 5 months and counting

My employer coverage ended at the end of the month and I am trying to work out what to do as an accountant. I have been trying to stay organised about it. I do not want a gap in coverage. Is there help filling out the forms?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
