Applying for food benefits after losing my job - first time posting

I got a notice asking for proof of income and I am unsure what to send. I am in Salt Lake City, in case local advice matters. There are 16 people in my household.

Take photos of everything you send and write down the date. I had to resend one page and the record helped.
