Applying for food benefits after losing my job (Columbus)

I got a notice asking for proof of income and I am unsure what to send. I have read a lot of threads here and could not find my exact situation. I have a final pay stub, a termination letter and my unemployment approval. What did you submit and how long did approval take?

I sent my last two pay stubs, the separation letter and a bank statement. I heard back in about 8 weeks, and my county offered an interview by phone.
