Applying for food benefits after losing my job (Chicago)

I got a notice asking for proof of income and I am unsure what to send. I am in Chicago, in case local advice matters. There are 12 people in my household. What did you submit and how long did approval take?

Thanks for writing this out. I went through something close to it last year, and it got easier once I kept a simple log of every call and form.

I sent my last two pay stubs, the separation letter and a bank statement. I heard back in about 14 weeks, and my county offered an interview by phone.
