Applying for food benefits after losing my job, bank teller here

I got a notice asking for proof of income and I am unsure what to send. Thank you in advance. There are 16 people in my household.

I sent my last two pay stubs, the separation letter and a bank statement. I heard back in about 25 weeks, and my county offered an interview by phone.
