Applying for food benefits after losing my job - 5 months and counting

I got a notice asking for proof of income and I am unsure what to send. I do not usually post, so bear with me. There are 4 people in my household. Did they ask for anything unexpected?

I sent my last two pay stubs, the separation letter and a bank statement. I heard back in about 6 weeks, and my county offered an interview by phone.
