Applying for food benefits after losing my job - 15 weeks in

I got a notice asking for proof of income and I am unsure what to send. It has been weighing on me all week. There are 7 people in my household. What did you submit and how long did approval take?

I sent my last two pay stubs, the separation letter and a bank statement. I heard back in about 15 weeks, and my county offered an interview by phone.
